Discontinuation of Activities and Data Destruction:

As of 3/31/2024, UpStream Carolinas terminated participation in the ACO REACH model in South Carolina, including the discontinuation of all marketing activities. In addition, UpStream Carolinas will archive REACH Beneficiary data it received and retain such data for the timeframes as required under applicable law. Note that UpStream will continue participating in ACO REACH in other markets as part of UpStream G.A.P.-Q.
